I, the Parent or Legal Guardian of the Model (Child) named on this form, hereby give my full consent for the Photographer to create photographic images of my Child. I grant the Photographer the right to use, publish, and display the Images for the following purposes, in any media, worldwide, and for an unlimited duration: Photographer's Portfolio: Inclusion in the Photographer’s printed and digital portfolios. Website and Social Media: Display on the Photographer’s professional website, blog, and social media channels (e.g., Instagram, Facebook) for the purpose of showcasing their work. Studio and Marketing Materials: Use as sample images in the Photographer's studio, and in promotional materials such as flyers, brochures, and email newsletters. Competitions and Exhibitions: Submission to photographic competitions, and for display in art galleries or exhibitions.
